INSTRUCTIONS AND SAMPLE COLLECTION:
Proper collection of a hair sample is critical to the success of your program so please read the "Instructions for Collecting a Hair Sample".
Ensure correct amount of hair for laboratory processing by using the by printing out a pattern of the "Hair Sample Weight Scale". Just trace the pattern on a standard office file folder, cut out, and fold the sides and you have an instant scale. Or if you have a digital jewelry scale you can weigh out 125 milligrams of hair. Roughly a teaspoon or two of hair is usually enough for a laboratory sample.
All hair analysis kits must be submitted with the applicable forms along with the Nutritional Balancing Academy Membership form.
SUBMISSION OF HAIR ANALYSIS KIT SAMPLE:
Cut your hair sample and put in a small white envelope. DO NOT PUT in aluminum foil, plastic baggie, rubber band or scotch tape the hair.
After payment mail the Laboratory Order form and your hair sample sealed in a clean small sample envelope to:
